Form loading multiples in Popup Template

I have an issue where a Popup Template that loads a script to create a form is getting called multiple times and creating multiples of the form. I have a link to the live and staging site, but can't post those in the forum, also have the script on a Gist, forum is restricting my post.

Have you seen this before? Any suggestions for fixing this issue?
