Need to collect data in a form and on submission an URL should be generated from form values and called

Hello, I need to prefill an external booking system. So I have to collect two dates, arrival and departure and finally the number of adults. Result should look like this:

?arrival=30.12.2021&departure=31.12.2021&adults=2

I have a form asking for the 3 field and on submission / clicking button, the above URL should be generated and be opened in a new tab.

Can you please guide me on how to archive this?
