Form not displaying properly

Hello all, I am having a problem with a form that appears on 4 pages of my website not displaying properly. The form is a section which is added to the pages using shortcode. I edited the column widths of the Forename, Surname, Email and Contact Number fields from 50% to 100% to see if it would look nicer. It did on the preview, but when published, the fields all appeared in the wrong order and the wrong size, like this:

So, I decided to change it back to how it was before. I am able to get it to appear like this in the editor, which is how I want it to look:

But no matter what I try, it will not appear like this on the page when published; it goes back to the first image. Sometimes when I refresh the page, the proper version appears momentarily - like it flashes on the screen before going back to the messed up version. When I click to edit the page, it comes up in the correct way as if nothing is wrong, but it won't appear this way on the published version.

I have I've tried editing in pages and the section editor, deleting and adding the section, deleting and adding the fields, changing the widths of the fields, even reverting the section back to old versions to try and undo everything and go back to the original, but nothing is working. Does anyone have any ideas why this is happening and how I may be able to fix it?
